The City’s Health Registrar shares office space with the City Clerk.
The Health Registrar records and issues birth and death certificates as well as executes voluntary paternities.
A birth or death certificate may be requested by mail.
A birth record will only be released to the individual (over the age of 18) on record or a parent listed on the record.
Death certificates will only be released to a spouse, sibling, parent or living child of the person named on the record. An attorney can always request any record on file on behalf of an authorized individual.
The cost for each certified copy is $10.00 payable to the Gloversville City Clerk.
To obtain these by mail you will need to supply the following:
- A photocopy of a picture I.D.
- A check or money order payable to the Gloversville City Clerk ($10.00 per copy).
- A self-addressed stamped envelope (#10 legal size).
